Coordinates patient care/collaborates with other health care providers Practices utilization management Completes patient documentation Actively participates in the quality improvement process Monitors trends in case management including processes and levels of care and proactively applies to work environment Performs data analysis
Position Requirements: 5 years clinical nursing experience in an acute care setting required 2 years of the above clinical experience with the population to be served in the designated CM position. Experience with Innerqual criteria and/or other systems strongly preferred Knowledge of CMS and third party regulations strongly preferred Strong interpersonal and communication skills Demonstrated ability to collaborate with patients and families, physicians and other disciplines Strong understanding of electronic information systems
Education Requirements: Graduate of an accredited school of nursing. Bachelors in Nursing preferred Participation in Case Management training preferred. License Requirements: RN with current license to practice in the state of Ohio CPR certified BLS Preferred Case Management Certification preferred in lieu of certification at time of hire, agreement to obtain certification within one year of meeting work experience requirement for certification Preferred Qualifications: Utilization Review Experience. Experienced with Denial and Appeals Management.
